The Young Person to Adult Transition Worker is integral role in transitioning people through their treatment from a young person’s specialist services into to adult services; working closely with the Young Person service collaborating in making the transition and change in service cultures as smooth and efficient as possible is key to ensure that we continue to meet the needs of young adults accessing support.

You will be responsible for a caseload of young adults affected by drugs/ and or alcohol with our Burnley Team.

You will always maintain high professional standards and ensure a service user focused approach in all aspects of delivery; You will lead by example in creating a professional and person-centred team culture focused on excellent service delivery.
