We’re looking for Homelessness Drug and Alcohol Recovery Practitioner Service
- In collaboration with the SABP clinical team you will ensure any alcohol and drug prescribing interventions are outcome-focused and fully integrated into recovery plans and are strengths based but linked to risk and care planning.
- You will undertake a wide range of ‘duty’ and ‘care-coordination’ functions, working with people from the point of first contact right through to their planned exit from the service following completion of treatment.
- Promoting recovery in their daily practice in both group and one-to-one settings.
- Completing comprehensive assessments, risk assessment, risk management and Recovery Care Plans with them to ensure personalised and recovery-focused treatment journeys.
- Acting as a care coordinator by providing a range of evidence-based interventions such as key working, motivational interviewing, ITEP mapping, harm reduction and relapse prevention.
- Working in partnership with our clinical and aftercare services, primary care services, Criminal Justice agencies, Young Peoples’ services, treatment providers and the local community.
- Carrying out regular Recovery Care Plan reviews, attend and co-ordinate multidisciplinary review meetings both within the service and with external parties where appropriate, and provide detailed reports for those involved with statutory services.
Maintaining accurate and timely records through the case management system in compliance with our Information Governance policy.
