Experts Explain the Difference Between PHP and IOP for Addiction Treatment

A guide to two common US outpatient addiction treatment programmes, explaining how they differ in intensity, structure and the support they provide.

Partial hospitalisation programmes (PHPs) and intensive outpatient programmes (IOPs) differ in time, structure and support. Each may suit a different stage of recovery. Weighing PHP vs IOP addiction care helps you, a loved one or a client ask sharper questions before choosing.

PHP vs IOP Addiction Care by Weekly Hours

Both programmes are outpatient care, so people attend scheduled treatment and return to their own living situation each day. Neither involves an overnight hospital stay. The main difference is the amount of time and structure each programme provides.

Medicare guidance used by programmes in the United States sets a minimum of 20 weekly hours of services for PHP and a physician-determined minimum of nine hours for IOP. IOP fills the gap between traditional once-weekly therapy and PHP or inpatient care for people with an acute mental illness or substance use disorder. Individual programmes set their own schedules, so these figures work as benchmarks.

PHP and IOP are U.S. levels of care and Medicare is the U.S. federal health insurance programme, which covers PHP for treating substance use disorder. Fewer hours reflect a different intensity matched to a different need.

What a PHP Treatment Programme Involves Day to Day

A PHP treatment programme is the more intensive of the two, built around near-daily clinical contact. Medicare describes it as a structured outpatient alternative to inpatient psychiatric care, with programmes usually involving four to eight hours daily of treatment. Covered services may include education about a condition and occupational therapy.

A typical day may include group therapy, individual therapy and education, though schedules vary. If you need more than weekly therapy but not round-the-clock supervision, PHP is designed for that middle ground.

Keeping a job during PHP can be difficult because treatment often takes up much of the day. IOP typically offers more scheduling flexibility.

What an Intensive Outpatient Programme Looks Like in Practice

Intensive outpatient care provides structured support through fewer, shorter weekly sessions. For instance, IOP clients at Diamond House Detox, a Northern California treatment provider, meet one-on-one with a psychiatric nurse practitioner, a substance use counsellor and master’s-level therapists, with afternoon and evening sessions available, according to the provider.

According to Vicky Magobet, a psychiatric mental health nurse practitioner and founder of Diamond House Detox, ‘Medical providers are also in-house, so your medication can be checked and adjusted.’ If sleep, mood or cravings shift, an in-house provider can review medication needs.

IOP can address co-occurring mental health and substance use concerns at the same time. Fewer hours can still include clinical oversight, and the right fit may change during recovery.

How Clients Step Down From PHP to IOP

Magobet explains that levels of care aren’t fixed and many people may move between them as their needs change. ‘PHP is often the right fit after residential treatment,’ Magobet notes.

Many clients start in PHP for added structure and step down to IOP as they progress, with the care team guiding the timing. For example, someone leaving residential treatment may attend PHP on weekdays. As your routine steadies, you shift to IOP evening sessions while returning to work.

Programme length depends on progress, goals and care needs. Stepping down can feel like losing support, but it still provides structured care with fewer weekly hours.

Matching a Level of Care to Where You Are Today

PHP offers more weekly hours and a near-daily structure as an alternative to inpatient care, while IOP offers fewer, shorter sessions with more flexibility and clinical oversight. Moving between levels is a normal part of recovery, guided by the care team. The right level of care is the one matched to where you are today and a clinical assessment can help you find it.
