Waythrough launched in October 2024 following the merger of Humankind and Richmond Fellowship.
Our vision is to break down the barriers that stop people getting the support they need to live a life they value.
This role will work as part of the Forward Leeds Young Persons Team to deliver drug and alcohol related interventions for young people (up to 21 years or 24 years depending on vulnerability) across Leeds. Work delivered will be outreach so based in schools, colleges, Youth Justice Centres and Community Venues.
Responsibilities will include:
- Case Management: Be the driving force behind the drug and alcohol treatment journeys of young adults. Conduct assessments, develop personalised care plans, and deliver interventions that truly make a difference.
- Community Promotion: Amplify our services within the community by building strong relationships with local organisations. Empower young adults and their families through engaging education and awareness sessions.
- Quality Assurance: Maintain the highest standards of care by implementing and adhering to service policies and procedures. Conduct thorough audits and keep meticulous records.
A full driving licence is required.
