As a Community Based Substance Misuse Nurse, working with Change Grow Live in Westminster you will coordinate and oversee client care for time-limited clinical interventions, supporting safe alcohol reduction and detoxification using validated tools and CGL care plans.

They will lead engagement with GP practices, arranging outreach clinics, training, and referral pathways, while also providing professional liaison with mental health and dual diagnosis services.

In addition, the role includes mentoring and supporting student and newly qualified nurses, as well as offering service cover across hospital and community settings when required.
