RECONNECT is a vital service designed to enable individuals leaving prison to access and sustain supportive interventions that improve their health, well-being, and chances of reintegration.
We work alongside service users to create bespoke care plans that address physical health, mental health, and wider support needs. Through strong partnerships with healthcare, probation, and community providers, we ensure a safe, person-centred transition back into the community.
We are seeking a motivated and compassionate Coordinator to join our RECONNECT team.
You will play a key role in supporting individuals as they prepare for release, ensuring they are connected to the right services and empowered to build healthier, more stable lives.
Key Responsibilities
- Receive, process, and triage referrals from prison healthcare teams, probation, and other partners.
- Act as the first point of contact, managing enquiries and ensuring referrals are appropriately prioritised.
- Build trusting, trauma-informed relationships with service users in custody and on release.
- Develop tailored care plans in collaboration with service users and partner agencies.
- Work closely with prison healthcare, probation services, voluntary sector, and community providers to deliver seamless support.
- Maintain accurate and up-to-date records on secure databases.
- Contribute to service monitoring, evaluation, and audits.
- Support the training and development of new RECONNECT staff and trainees.
- Share best practice, engage in risk assessments, and work collaboratively with colleagues to ensure safe, high-quality service delivery.
