Would you like to work in an outreach capacity to support our most vulnerable residents into treatment?
Greenwich has a dynamic and experienced team who would love to hear from you.
We are looking for a dynamic and driven individual who is committed to providing assertive outreach to those who use our services.
Could you help engage or re-engage the homeless population into wellbeing services and drug and alcohol treatment.
You will be able to work both independently and as part of a supportive team to:
- to carry out triage and comprehensive substance misuse assessments, risk assessments and develop person-centred Recovery Care Plans predominantly with them.
- to conduct street outreach to engage/re-engage homeless individuals misusing substances.
- to conduct assessments at local hospitals and liaise with their substance misuse team on a daily basis.
- to develop strong links and partnerships with a wide range of community groups aimed towards vulnerable people to encourage and promote access into services for people.
- to deliver satellite provisions within local services e.g., GPs, probation, hostels etc.
- to deliver harm reduction advice, well-being, education, and support.
- to work in collaboration with partner agencies in housing, immigration, healthcare, etc.
