We have an exciting opportunity to join our Criminal Justice team in Devon as a transitions worker.
This role is a new, innovative way of working, one in which supports those who have been in prison to resettle back into the community.
We are looking for a new transitions worker to join our already established and experienced team in continuing to build on this way of working.
The job involves working closely with our local prisons and its substance misuse teams to identify people who may need support with their substance use upon release.
We work holistically with many partners to ensure the service users we work with have the best chance in reintegrating into our communities, reducing offending, reducing harm to others, as well as supporting them achieve their recovery goals.
