Description
The Senior Practitioner, reporting to the Locality Manager in Mansfield, will support the operational leadership for the service within the community setting, working with senior members and a range of key stakeholders and partner organisations to ensure a shared achievement of goals.
The post holder will work as an integral part of the multi-disciplinary team at the Mansfield Team, to deliver high-quality provision to service users.
As a Senior Practitioner, the post holder will focus on the quality and will be expected to work with service users via care coordination and act as a lead within the Mansfield Team.
Change Grow Live Nottinghamshire provides a range of intervention packages and programmes aligned to enabling individuals to ‘change,grow and live’. These include harm reduction, psychosocial interventions, recovery coordination and release planning and throughcare.
