Description
As a Personal Health and Wellbeing Practitioner, you will provide a person centred practical and emotional support service with therapeutic interventions which is responsive, and caring.
You will create an enabling environment and service for the given residents/participants in line with their agreed care plans and risk assessments, offering practical based care, ensuring that the service offered promotes the rights of the individual which preserves dignity, choice, privacy, independence, fulfilment and their personal goals.
