We’re looking for a compassionate and organised M-PACT Co-ordinator/Family.

Worker to lead the delivery of our M-PACT (Moving Parents and Children Together) programme and co-ordinate our existing family work initiatives within an integrated healthcare framework.

You’ll ensure the safety, confidentiality, and care of vulnerable adults and children, working closely with partner agencies to meet targets and measure meaningful outcomes. Collaboration and communication are at the heart of this role.

What we’re looking for:

• Experience supporting families or working in a health or social care setting
• Strong organisational and partnership-working skills
• Commitment to safeguarding and confidentiality
• Flexibility to work some evenings/weekends and travel across East Kent when needed
