Integrated Care Practitioner

We’re looking for a passionate and committed individual with a particular interest in working with high intensity users of hospital, mental health, 999, GP and Primary Care Networks, where support for alcohol and/or drug use has been identified as a need. If you’re looking to:

• Create simplified services for people with multiple and complex needs
• Work within multidisciplinary teams, ensuring that clients receive the right care at the right time
• Deliver services within a range of health and social care settings
• Be part of a team at the cutting edge of new ways of working
