We are excited to offer the opportunity for a Peripatetic Front-Line Navigator to join our Calderdale Recovery Steps Team.
This is a dynamic and rewarding role working flexibly across Todmorden and Halifax Hubs, where you will:
- Be person-centred, visible, and responsive, providing a warm and welcoming experience for the people who use our service.
- Facilitate and promote the progression of individuals from initial engagement into structured treatment, supporting treatment access at the earliest opportunity.
- Provide comprehensive assessment and respond to individual need as they present.
- Contribute to reducing substance-related harm to individuals and the wider community.
- Collaborate closely with key partners and colleagues to achieve the best health and wellbeing outcomes for individuals.
- Have Excellent boundaries and ability to apply ‘co-production’ principles to any stage of treatment and recovery.
- Be Able to think creatively about treatment options, changes and transitions in the context of a multi-disciplinary and multi-agency approach.
- Encourage service users to become involved in the service and in service user forums in Calderdale that will enhance their experience and increase a sense of belonging.
- This is a non case holding role but you will be expected to support case management tasks for the people you see, also covering sickness and annual leave as and when required.
By working diligently and proactively, as part of an integrated multi-agency team, you will apply intensive and co-created recovery orientated and Harm reduction interventions , enabling health & well-being improvements that are meaningful and sustainable.
