You will work as part of a team to provide collaborative support to service users and their families across all stages of recovery, supporting engagement and positive relationships.
To provide holistic and structured case coordination and psychosocial interventions to families, carers and service users. To support families and service users across all stages of custody including transition planning for transfer or release. Working to ensure effective delivery of care within the wider Reconnected to Health service model.
As a Family Support Worker, you will play a vital role in supporting families to achieve their goals. You will use a variety of interventions, tailored to meet the specific needs of each family, which will make a real difference to their lives.
Working as part of a Multi-Agency team, you will collaborate with schools and Children's services to ensure a joined-up approach to achieving the best outcomes for families.
