Our Connecting Communities Coordinators will support men with current or previous substance misuse history who are leaving HMP Altcourse for Cheshire, up to 12 weeks pre-release.
As a Coordinator, you will act as a named point of contact for men – focusing on individuals only having to tell their story once. Men showing willingness to engage in the programme will be encouraged to apply for a Rehabilitation to Resettlement (RTR) house.
As a Connecting Communities Coordinator, you will develop a Pathway Plan for prison leavers, ensuring accommodation is secured and any healthcare and/or psychological interventions organised before their release. The plan will introduce the man to their named contact in the community. The plan will mitigate high-risk situations for the individual, shared with RTR Housing Support staff to help prepare them for reintegrating and rehabilitating.
A drivers licence is essential for this role and Prison Vetting will also be required.
